2012 Honor Choir Registration

 

Congratulations to all of the fine singers who have been accepted for one of the 2012 ACDA Eastern Division Honor Choirs. Making one of these choirs is a huge honor and a big responsibility. This will be an event that you will never forget!

It is now time to:

  1. Accept your place in the choir by signing and sending in the Expectation Form. Please use the form that applies to your choir. And please retain a photocopy of the signed form for your reference.

  2. Sign and notarize the Medical Release Form. The medical release form must be signed in the presence of a notary public AND notarized. Please use the form that applies to your choir. You may wish to retain a photocopy of this signed form for your reference.

  3. Pay the registration fee. Two payment options are available:
     
    a.  Provide the information requested at right and click the "Add to Cart" button to pay using PayPal;


    - or -
     
    Student's Choir
    Student's Name
    b.  complete this form, and mail the form and a check made payable to "ACDA Eastern Division" to Penelope Cruz at the address below.

All forms must be completed and mailed and registration fees paid NO LATER than Oct. 1, 2011. Failure to meet these deadlines could result in losing your place in the choir. Once we receive registration fees, we will mail music to each teacher/sponsor (on or before November 1).
