2010 Honor Choir Registration

 

Congratulations to all of the fine singers who have made one of the 2010 Honor Choirs. Making one of these choirs is a huge honor and big responsibility. This will be an event that you will never forget!

It is now time to

• accept your place in the choir that you have made (fill out the Registration Form),
• pay the registration fee,
• sign the Expectation Form,
• sign and notarize the Medical Release Form, and
• begin making plans for the hotel.

Since all future correspondence will come by email to you AND your sponsor, it is VERY IMPORTANT that we have a current and correct email for both of you. Please make sure that you fill in the email section of the registration form so that we may verify it with the one we have on file.

Please note: All registration forms, expectation forms, and medical release forms must be completed and mailed NO LATER than September 21, 2009. All payments must be received by October 15, 2009.  Failure to meet these deadlines could result in losing your place in the choir. Once we receive all monies from registrants, we will mail music to each teacher/sponsor (on or before November 1).

The following items must be submitted to Stuart Younse:

1. Honor Choir Registration form. (This is an online form, and will be submitted to Stuart Younse automatically when you click "Submit" after completing the form.)
2. Payment
   • You may pay either using Paypal (the PayPal button is on the payment form page)  - or -
   • Send a check made payable to "ACDA Eastern Division" to Stuart Younse. Please write the singer's name(s) on the check AND include a copy of the Payment Form.

The following forms must be signed and mailed to the Honor Choir chairperson:

1. Expectation form
2. Medical Release form
   NOTE: The medical release form must be signed AND notarized. If the form is not notarized, the singer WILL NOT be able to participate!

Singers staying with parents will also need to make their own reservations at one of the Marriott hotels.  Reservations can be made at the special ACDA Conference rates by following the Marriott link available on each of the Honor Choir Registration pages.

Singers being housed with other choir members will fill out the Housing Request Form.  They will subsequently be contacted by the Marriott to register.
